BSA Medical Form
This form must be completed by every scout annually and turned into the troop. This is especially important for Summer Camp, and any campout that is longer than 72 hours requires the entire form to be filled out and a section completed by the scouts doctor.
Monthly Campout Permission Slip
This form needs to be completed prior to attending the monthly campouts. It gives permission for the scout to attend the campout and serves as the Troop's way of knowing you plan to attend. Both the scout and parent needs to sign the form and then turn it in with payment, ideally 2 weeks prior to the campout.
